Abstract:
A surgical stapling or fastening instrument for applying surgical fasteners to tissue having an adjustable closure mechanism to linearly approximate the distance between the jaw members of the instrument. The adjustable closure mechanism consists of an advancing mechanism and a retaining mechanism, wherein the actuating mechanism is actuable to urge the jaw members towards each other. A coupling arrangement is also provided which permits firing of the staples or fasteners only when the jaw members are approximated a predetermined distance from each other.

Abstract:
A surgical apparatus for securing a needle extending therethrough includes a post having a channel therein mounted to a base. The post is positioned in operative alignment with the needle mounted on the base. A locking member is associated with the post in operative alignment with the needle. The locking member defines an opening therethrough to receive the needle, and the locking member is operable between a needle receiving position wherein the needle may be inserted or removed from the locking member and a needle retaining position wherein the needle is retained in at least one direction of movement. A method for surgically localizing and removing tissue includes the steps of providing a tissue cutting member mounted on a base and defining an opening near a distal end, the tissue cutting member further forming a tissue receiving cavity in communication with the opening, a needle disposed within the cavity and defining a longitudinal passageway therethrough, a post mounted on the base and a locking member mounted on the post and defining having a needle receiving opening in operative alignment with the needle, the locking member being operable between a needle receiving position wherein the needle may be inserted or removed from the locking member and a needle retaining position wherein the needle is retained in at least one direction of movement, locking the needle within the locking member by rotating the locking member into the needle retaining position, positioning the needle within target tissue, severing the tissue to be removed and removing the severed tissue from a patient.

A surgical stapler is disclosed which includes a first body portion supporting an anvil plate defining a fastener forming surface, and a second body portion configured to releasably mate with the first body portion. A disposable loading unit is removably supported in the second body portion and includes a cartridge defining a plurality of slots, a plurality of surgical fasteners disposed in the slots, a plurality of ejectors positioned adjacent the surgical fasteners, and a wedged actuator configured to enter into and translate through the cartridge to sequentially interact with the ejectors. An elongated actuation member is mounted for longitudinal movement within the second body portion and is releasably engagable with the wedged actuator, whereby longitudinal movement of the actuation member causes the wedged actuator to interact with the ejectors, driving the surgical fasteners from the cartridge to be formed against the anvil plate.

An apparatus for applying surgical clips through an endoscopic tube includes an endoscopic portion housing a jaw blade, a pusher bar for feeding clips one at a time to the jaws, a channel bar for camming the jaw blades into closed position, and an array of surgical clips. A surgical apparatus for removing tissue, which includes a housing, an elongated body which extends from the housing and forms an opening at a distal end, the elongated body further forming a tissue receiving cavity in communication with the opening, a cutting member operatively associated with the housing and movable transverse to the elongated body in proximity to the opening, and a tissue retaining member positioned in proximity to the opening and the cutting member, the retaining member being selectively movable from a retracted position to a deployed position, wherein when positioned in the deployed position, the tissue retaining member obstructs at least a portion of the opening at the distal end of the elongated body. A method is also provided for surgically removing tissue including the steps of positioning a tissue removing instrument including an elongated housing having a tissue receiving cavity at a distal end, a first tissue cutting surface longitudinally movable relative to the elongated housing distal end, an obturator having a sharpened distal end portion such that the sharpened end portion is positioned adjacent the tissue to be removed and a second tissue cutting surface transversely movable relative to the elongated housing, removing the obturator from the elongated housing, coring the tissue to be removed, severing the cored tissue from the surrounding tissue with the second cutting surface, and removing the severed tissue from the patient.

An apparatus is disclosed for endoscopic application of surgical staples adapted to attach surgical mesh to body tissue in laparoscopic hernia surgery. The apparatus includes a frame, and a generally elongated endoscopic section connected to the frame and extending distally therefrom. A staple storage magazine is provided for storing a plurality of surgical staples at the distal end portion of the endoscopic section with each staple being configured and adapted to attach the mesh to the body tissue. An elongated pusher system formed of several assembled components and extending from the frame to the endoscopic section is provided for individually advancing at least one staple at a time distally for positioning adjacent the surgical mesh and the body tissue. The pusher system also includes a trigger mecanism to actuate the pusher. Anvil means provides for closing the staple to encompass at least a portion of the surgical mesh and to penetrate the body tissue in a manner to attach the portion of the mesh to the body tissue.
